Lease/Option Forms

I am new to investing and would appreciate a copy of a lease/option to purchase agreement. I was recently at a conference where someone expressed that David Conti had a good agreement. Does anyone have a copy they can share?

    I think they sell their forms for $397, but without any training on how to fill them out, it might be a bad investment.
