Contact the secretary of State in whatever state you wish to form the LLC. The initial form is pretty easy to fill out. Its the operating agreement that is more complicated-- but you don't have to file it with anybody.

In addition there is probably a book at the bookstore called "How to start a business in Florida" (or where-ever) which may give more detailed instructions.

Mark Warda(use to practice law)has a book you can buy from www.amazon.com or go to www.campusi.com search by author for for his "forming a LLC in florida" book with all the forms+xtra stuff for under $20.

You can find LLC forms at just about any office stationary store, books on this at the public library and by clicking on TCI's product or services buttons at the top of TCI's page.

Cost just depends on what service or package you would choose.

In Florida, you can do it online at www.sunbiz.org.
Total cost is $160 which includes (optional Certified Copy and Certification of Status, a $35 value).
