Anyone Have A Good RFQ Form?

I need to have some work done on a rehab property, and it's a case where I don't want to use my usual contractor. I would like to send a Request for Quote out to some of the local companies in the Yellow Pages, but don't have a good form in my 'forms library'. If anyone could provide me with a good RFQ template I would appreciate it.

    I have always just called, explained what I needed done, and set a time to personally meet them at the property.

    From there I let them go about their way to determine a bid but also get a feel for the ethics of the contractor. I am a very good judge of character. I prefer to go by referal rather than respond to an ad in the yellow pages or newspaper.
